A bicornuate uterus forms as a result of abnormal intrauterine development of the fetus. This pathology represents an abnormal division of the uterus into two sections and is quite common – occurring in 3% of all women. Among the causes of the condition is the abnormal development of the Müllerian duct, the organ from which female reproductive organs are formed. In 50% of cases, a bicornuate uterus does not present any symptoms.

Women with this pathology are considered to be at risk. Most of them have an increased risk of spontaneous abortion (miscarriage), with the likelihood of successful births being around 63%. There is a risk of premature births (15-25%), and in 40-50% of cases, children of such patients are delivered feet first, which poses complications for both the mother and the child. There are documented cases in medical history where a bicornuate uterus has led to the development of uterine tumours.

Treatment of bicornuate uterus – minimally invasive surgeriesOn the other hand, in some cases, a bicornuate uterus does not pose any obstacle to successful conception, carrying, and delivery of a child. For instance, in the early 2010s, a 40-year-old patient with this anomaly successfully gave birth to twins in a hospital in Madrid – an ultrasound showed that she had a bicornuate uterus with a twin in each "horn". At 38 weeks, she underwent a bilateral cesarean section, and healthy babies were born. Such a fortunate outcome is rare, so the gynecologists at our clinic still recommend surgical treatment for patients – this measure can significantly reduce the risks associated with fertility and reproduction. Additionally, children of patients with this pathology have a fourfold higher risk of developing congenital anomalies.

  • The main method for correcting this anomaly is metroplasty according to Strassman’s method – a surgical operation to restore the normal, physiologically natural shape of the uterus. The operation is performed under general anesthesia, during which an incision is made along the midline of each "horn" to the uterine cavity, the dividing septum is removed, and then the edges of the myometrium are sewn together to form a single, undivided uterus. Statistics show that after metroplasty according to Strassman, about 83% of pregnancies end in successful births (there are variations of this operation – metroplasty according to Jones and Tompkins, differing in the specifics of suturing the uterus).
  • Often, the optimal treatment method is hysteroscopic resection – a minimally invasive operation for laparoscopic removal of the septum dividing the uterus into two sections (including using laser instruments and electrocautery). The advantages of bloodless laparoscopic surgery include reduced blood loss, good control of the intervention by the surgeon (minimal risk of perforation of the uterine walls and damage to the intestines), and a minimal likelihood of adhesion formation. Furthermore, the operation is characterized by a good cosmetic effect – only a few scars less than 1 cm long remain on the abdomen after it.
  • Regardless of the method of correcting the bicornuate uterus, after the operation, the patient undergoes a course of hormone therapy, aimed at normalizing hormonal balance.

  • Uterus removal: saving the patient's life

Diagnosis of Bicornuate Uterus in Israel

In most cases, the diagnosis of a bicornuate uterus in Israel is conducted within a short time frame, taking 3 to 4 days. During this time, the patient undergoes all the examinations prescribed by the gynecologist and receives a treatment plan.

First Day – Meeting with the Leading Gynecologist

A representative of the clinic meets the patient at Tel Aviv airport, and together they head to the Top Ihilov medical center for a consultation with the gynecologist. The doctor conducts a primary examination, studies the medical history, and plans a series of diagnostic tests.

Second Day – Diagnostic Procedures

  • Sonohysterography – a special type of ultrasound diagnosis that allows for the examination of the uterine cavity for internal anomalies.
  • Hysterosalpingography – a radiological method for examining the uterine cavity. During this type of diagnosis, a radiopaque substance that is not transparent to X-ray radiation is introduced into the uterine cavity, followed by fluoroscopy.
  • Hysteroscopy – examination of the uterine cavity using endoscopic equipment. This type of diagnosis may sometimes be combined with surgical treatment of the bicornuate uterus (hysteroscopic resection).
  • MRI – a highly sensitive and accurate method that allows for the visualization of all uterine anomalies in three-dimensional space.

Third Day – Planning the Corrective Surgery

On the third day, after establishing the final diagnosis based on the results of the conducted studies, the leading gynecologist determines the type of surgical correction and plans the stages of the operation.

  • Magnetic Resonance Imaging (MRI)
  • X-ray
