About the Disease

Chondropneumonitis of the larynx is a pathology caused by infectious diseases, characterized by the penetration of purulent flora into the perichondrium. Without adequate treatment, chondropneumonitis can take an acute form and may then progress to a chronic stage.

Primary perichondritis develops in infectious diseases such as measles, influenza, and others. Secondary forms of the disease include lesions of the laryngeal mucosa and damage to the perichondrium with cartilage. These pathological processes develop in syphilis, cancer, tuberculosis, and most often due to various traumas. Additionally, there is radiation chondropneumonitis, which develops during or after radiation therapy for malignant laryngeal tumours, and toxic chondropneumonitis, which occurs due to mercury poisoning.

There is also a conditional division into external and internal chondropneumonitis depending on where the pathological process predominates – on the outer or inner surface of the cartilage and perichondrium. Most often, both layers of the perichondrium are affected.

Medication Treatment

Antibiotics in combination with sulfonamides are used to treat chondropneumonitis of the larynx. In cases of severe pain, inhalations or lubrication of the larynx with dicain are also performed. Intranasal novocaine blockade yields good results. The patient is given a semi-liquid food in a puree form. In cases of severe dysphagia, nutritional enemas are prescribed. The use of a gastric tube is not recommended as it will be a constant irritant.

Surgery

In cases of internal perichondritis, decongestants are prescribed to prevent the development of asphyxia, and if laryngeal stenosis increases, a tracheotomy is performed. If a limited abscess is present, it is opened with a laryngeal knife endolaryngeally, allowing for the removal of any freely lying necrotic tissue.

If such interventions are ineffective, an external operation is performed: resection of the thyroid cartilage with submucosal opening of the abscess, without, incision or splitting of the larynx, with scraping of granulations and fistulas, or with wide submucosal excision of all affected cartilages. If there are indications for laryngeal opening, it is advisable to perform laryngotracheostomy, i.e., to incise the larynx and the upper rings of the trachea, suturing their mucous membrane to the skin.

Radical types of surgical treatment for this disease are not always advisable. If the inflammatory foci are small and well-defined, chondropneumonitis of the larynx can be treated conservatively or conservatively surgically.
