Treatment Methods for Mediastinal Lymphoma

Primary large cell mediastinal lymphoma grows quickly and requires prompt treatment. The main goal is to eliminate all signs of lymphoma and achieve stable remission. The treatment approach depends on the stage of lymphoma and the presence of symptoms causing problems. The most common treatment methods are presented below.

Immunotherapy and Chemotherapy

Mediastinal lymphoma is often treated with a combination of the immunotherapeutic drug rituximab and chemotherapy. This is called chemoimmunotherapy. Commonly used combinations of chemoimmunotherapy for PMBCL include:

  • R-CHOP
  • DA-EPOCH-R – etoposide, prednisone, vincristine, cyclophosphamide, doxorubicin, and rituximab. “DA” means “dose-adjusted”. This means that the dose of medications can be adjusted based on how your body responds to treatment.

Steroids

Steroids are medications that are often prescribed alongside chemotherapy for treating lymphomas. They help make chemotherapy more effective.

Radiation Therapy

Radiation therapy uses high-energy beams to destroy cancer cells while causing as little harm as possible to normal cells. Some patients are prescribed radiation therapy after chemotherapy and rituximab for treating mediastinal lymphoma. This method can also reduce the risk of lymphoma recurrence in this area.

Stem Cell Transplantation

Stem cell transplantation is used to treat lymphoma in case of relapse. Stem cell transplantation is an intensive treatment method, so it is not suitable for everyone. The transplantation can be done using the patient's own stem cells – autologous stem cell transplant – or cells from a donor transplant – allogeneic stem cells.

CAR-T Cell Therapy

CAR-T cell therapy is sometimes used to treat relapsed mediastinal lymphoma. CAR-T therapy (chimeric antigen receptor therapy) involves collecting the patient's own immune cells and reprogramming them. The cells are then used as treatment to target lymphoma cells. CAR-T therapy is also used when lymphoma progresses and when other treatment methods have not helped.

Diagnostic Methods for Mediastinal Lymphoma

A comprehensive examination at Ichilov Clinic, which allows identifying all causes of the patient's poor health and making an accurate diagnosis, usually takes place in three stages and lasts three days, not including the time needed to complete some laboratory tests.

Initial Consultation

The patient visits a specialist doctor in the diagnostics department. He conducts a medical examination, reviews the medical history, and refers the patient for laboratory and instrumental studies. If the patient has bone marrow biopsy materials, they are sent for review.

Diagnostic Tests

The next morning, the patient undergoes a blood laboratory test.

Then a PET-CT or similar imaging studies (CT, MRI) are performed. This instrumental examination helps determine the stage and extent of cancer.

The most common test for confirming the diagnosis is a biopsy. The doctor will take a tissue sample from the affected area. Most often, a biopsy is taken from an enlarged lymph node.

All procedures and consultations are organized by a medical coordinator, who also provides translation into Hebrew.

Consultation, Treatment Planning

When the examination results are ready, the attending physician gathers an expert commission consisting of doctors who will participate in the treatment process. These are experts from related fields of medicine – oncologists, thoracic surgeons, radiologists, chemotherapists, bone marrow transplantation specialists. The doctors will collegially discuss the clinical case and develop an individualized treatment plan that best meets the patient's needs. After the consultation, the attending physician will discuss the advantages of the chosen methods with the patient and inform him of the possible risks, as well as provide an overall prognosis.

  • Biopsy
  • Computed Tomography
  • Magnetic Resonance Imaging (MRI)
  • Diagnosis – PET CT
