Treatment of Extrahepatic Bile Duct Cancer in Israel Using Modern Surgical and Conservative Methods

For decades, Israel has confidently ranked among the leaders in the treatment of oncological diseases. The high qualifications of doctors and the state-of-the-art equipment in clinics allow for the diagnosis and treatment of various types of oncopathologies, from the earliest stages to terminal stages. Treatment of extrahepatic bile duct cancer in Israel is conducted according to an individualized comprehensive program that includes the most effective modern methods. Surgical operations are performed using minimally invasive techniques, characterized by low trauma and rapid recovery. Chemotherapy is administered using the latest generation of drugs.

In cases of oncopathologies of the bile ducts, early diagnosis is complicated by the fact that, most often, painful symptoms do not manifest in the initial stages. The diagnostic equipment in Israeli clinics allows for the detection of tumours of the smallest sizes and determining their characteristics, which are necessary for selecting the most effective therapy methods and drugs. According to statistics, early diagnosis and treatment using progressive techniques ensures the formation of stable remission in more than 85% of patients. At the same time, the cost is lower than in Europe and the USA, making it accessible for medical tourists.

Treatment Methods for the Disease

Oncopathologies of the extrahepatic bile ducts (cholangiocarcinomas) are relatively rare, accounting for no more than 2% of all types of malignant diseases. The disease is characterized by aggressiveness and relatively rapid spread; however, most patients do not exhibit characteristic painful symptoms in the early stages. The treatment plan depends on the stage of the disease, the location of the tumour, and a number of other factors.

Surgical Treatment

The most effective treatment method involves the removal of the cancerous formation through minimally invasive surgical intervention. Depending on the stage of the oncopathology, surgeries may be performed to completely remove the tumour or to alleviate painful symptoms, improve the quality of life, and prolong the patient's life (palliative procedures).

The type of surgical treatment depends on the location of the tumour and the extent of the malignant process. In the early stage, the affected bile duct is removed along with regional lymph nodes. In later stages, the gallbladder is removed along with the duct, partial hepatectomy (removal of part of the liver) may be performed, or a Whipple procedure may be conducted.

In Israeli clinics, surgical interventions are performed using minimally invasive laparoscopic or endoscopic methods, with a low likelihood of bleeding and postoperative complications, and a short and uncomplicated recovery period.

Photodynamic Therapy

A progressive technique that involves destroying malignant cells through laser irradiation. Before the procedure, a photosensitizing drug is administered to the patient, which accumulates in cancer cells, significantly increasing their sensitivity to laser radiation. With precise laser irradiation, malignant cells are irreversibly damaged and die.

Radiotherapy

The comprehensive treatment program for oncopathologies of the extrahepatic bile ducts includes courses of external or contact radiation. External radiotherapy is conducted on modern Novalis TrueBeam STx systems, ensuring accurate irradiation of the malignant focus and minimal negative impact on healthy tissues. Brachytherapy — a method of introducing a capsule with radioactive isotopes directly into the tumour tissues — is characterized by high efficacy and safety. The high-dose irradiation produced from within the tumour destroys cancer cells, provides a favorable prognosis, and practically does not harm surrounding tissues.

Radiotherapy is usually used in conjunction with surgical treatment and chemotherapy. A course of radiation therapy is prescribed before surgery to reduce the size of the cancerous formation and facilitate its subsequent removal. After surgery, irradiation is indicated to destroy remaining cancer cells and prevent recurrence. If a patient has reasons that make surgery impossible, radiotherapy is prescribed along with chemotherapy as an independent treatment method.

Chemotherapy

A chemotherapy course is indicated after the surgical removal of the cancerous formation to destroy malignant cells that remain in the patient's body and to prevent recurrence. Chemotherapy administered in cases of inoperability or diagnosed recurrence of the tumour aims to prevent the spread of the malignant process. Israeli clinics also widely use HIPEC — hyperthermic intraperitoneal chemotherapy. This procedure is performed at the final stage of surgery and involves washing the abdominal cavity with a concentrated solution of chemotherapy drugs at high temperatures. Heating the solution to 42℃ improves penetration into pathological tissues, while local exposure significantly reduces damage to healthy cells and minimizes side effects.

Patient reviews of chemotherapy in Israeli clinics confirm the use of modern chemotherapy drugs, which are much less toxic than traditional ones and cause minimal pronounced side effects.

Targeted Therapy

An innovative method based on the use of drugs that specifically target cancer cells without affecting healthy ones. In the treatment of oncopathologies of the extrahepatic bile ducts, targeted drugs from the group of angiogenesis inhibitors are used. Their mechanism of action involves preventing the formation of blood vessels responsible for supplying the tumour, leading to the destruction of pathological tissues and reduction of neoplasia.

How the Diagnosis of the Disease is Conducted

The use of high-tech equipment in Israeli clinics allows for the diagnosis and determination of the characteristics of the tumour with maximum accuracy. In most cases, this takes no more than three days.

First Day

During the initial consultation, the treating oncologist reviews the patient's medical history, conducts a physical examination, and prescribes necessary tests.

Second Day

Completion of prescribed tests, including:

  • general and biochemical blood tests;
  • ultrasound;
  • visual imaging methods (CT, PET-CT, MRI);
  • endoscopic retrograde cholangiopancreatography (X-ray examination of the bile ducts);
  • biopsy;
  • angiography.

Third Day

A medical council consisting of the treating oncologist and several specialized experts reviews the results of the tests and develops a comprehensive treatment program based on them.

What is the Cost of Treatment for the Disease

The final cost is determined after the diagnostic examination. Overall, the price of treatment in Israeli clinics is 30-50% lower than in medical centers of comparable level in Western European countries and the USA.

Advantages of Treatment in Israel

  • High level of qualifications and experience of doctors, many of whom are recognized worldwide.
  • Accurate diagnosis on state-of-the-art equipment.
  • High efficacy of the treatment methods used.
  • Comfortable conditions for undergoing treatment.
  • Affordable prices.

With timely diagnosis and therapy, the cure of oncological diseases of the extrahepatic bile ducts is possible. The main thing is not to lose precious time and hope and to start restoring your health as soon as possible.
